Prior To approval
- Employer Information form
- Job Description
- Copy of Workplace Liability Insurance
Once Approved
- Signed Subsidized Placement Agreement
- Copy of the Offer of Employment
Halfway Through the placement
- Copy of Health and Safety regulations or training provided
- Online meeting to receive feedback
At the end of the placement
- Invoice for the amount of the agreement along with supporting documentation)
- Evaluation Questionnaire
- Testimonial for website
